Mobile Notary Health Screening Questionnaire

Protect The Notary, The Client & The Witnesses With These Three Questions


As concern over the coronavirus continues to grow, AZ Roving Notary takes Covid-19 precautions very seriously for your clients, as well as for our own employees and family.

We adhere strictly to the CDC and National Notary Association health and safety recommendations and guidelines.

Our notaries, your clients and witnesses are asked to wear a mask at all signings. We bring hand sanitizers for those clients who don’t want to wear gloves. We sit at the safest distance available, and our pens are sanitized with Clorox wipes at the end of every signing. Weather prevailing, we can even have the documents signed, witnessed and notarized outdoors.

On every mobile notary assignment accepted, we always perform a brief screening questionnaire with the signer prior to the signing appointment.

Before you send a notary to your client's home or business, you should ask the following questions of each participant:

1. Have you or any of your household returned from domestic or international travel outside of Arizona within the last 14 days?
2. Have you or any of your household had close contact with or cared for any one with the Covid-19 virus within the last 14 days?
3. Have you or any of your household experienced any cold or flu-like symptoms within the last 14 days. (Fever, cough, sore throat, respiratory illness, difficulty breathing, loss of taste, loss of smell)

If the answer is yes to any of the three questions, it is not recommended that you send a mobile notary to the client. A remote online notarization may be the solution for clients who can’t answer the screening questions with a solid no.
